
A SIMPLIFIED CHECKLIST
â–¡ READ: Carefully read all the info on this page.
â–¡ GATHER: Gather information for ALL students & for any adults staying on campus.
â–¡ REGISTER: Use the gathered information to register students and adults online NO LATER THAN May 3.
â–¡ PROVIDE: Provide blank medical forms to every student and adult attending.
â–¡ SCAN: Gather completed medical forms, scan them, and email them as a combined document to yopaul@gmail.com by May 9.
â–¡ PAY: Send a church check made out to "WVS NYI" to Paul for full payment for students and adults by May 9. (Address is near the bottom of the page.)
â–¡ PREPARE: Prepare yourself and your students for an awesome time at BLAST!

How to Register

ALL ADULTS AND STUDENTS who are attending the event must be registered by May 3, 2025. You can use the form LINKED HERE ONLY to gather the information from your students before completing their online registration. The paper version of this form is ONLY for your use and will not be turned in. Here's the information you'll need to gather before registering:
FOR ADULTS:

6) shirt size
7) whether they want housing/meals during the event.
FOR STUDENTS:
1) name

YOUTH LEADERS: Please DO NOT put your own email address and cell phone for students. This will prevent students/parents from receiving necessary information.

IMPORTANT: If a student wants to participate in an event that they did not participate in at District BLAST, contact yopaul@gmail.com before registering them. (We only have five spots for each event. This does not apply to events that are open at Field BLAST.)

COSTS & PAYMENT: Fees should be paid to the WVS district NYI as one payment from the church. For more information regarding fees and housing, you can read this PDF, but basically, fees are broken down into three categories:

Registration: Student - $80, Adult - $40

On-Campus Housing (2 or 3 nights = same cost): Student or Adult - $30

Cafe Meal Package: Student and Adult - $50 (This includes all three meals Thursday, all three meals Friday, and breakfast on Saturday.)

MEDICAL/RELEASE FORMS are required for every student and adult. The completed medical forms should be scanned into a PDF packet and emailed to yopaul@gmail.com by May 9. This will enable us to organize them and provide them to the university, since MVNU requires that the forms be present for students and adults to be on campus. For students, these forms must be signed by the parent/guardian. The blank form can be downloaded here: Medical and Civil Liability Release Form​
